at a convenient point or points within the coal mining fields, not less than
two complete resuscitating outfits and two full rescue outfits, of type
recommended by the United States Bureau of Mines and approved by the
Bureau, which shall be available for instruction of rescue crews and for
immediate use whenever needed in rescue work.

165. The operator of every gaseous coal mine employing as many as
one hundred men underground shall endeavor to have trained for rescue
work as many as three crews of four men each from his employees; and
in such mines employing less than one hundred men, at least one such
crew of four men; instructions and facilities to be provided for such crews
by the Bureau, and such crews to be permitted to use the apparatus of
the Bureau for training purposes under regulation of the Bureau.

166. The operator of each coal mine shall report monthly to the district
mine inspector on blanks provided by the Bureau, such information as the
Bureau may require regarding all fatal and serious accidents that have
occurred in and about such mine.

167. Whenever any district mine inspector receives notice of the
occurrence of a fatal accident occurring at any mine in this State, it shall
be the duty of such inspector to go immediately to such mine, to inquire
fully into the cause of the accident and to report thereon to the Bureau in
writing; a copy of which report shall be sent to the operator of such mine.
To enable the district mine inspector to make such inquiry and report,
he shall have power to require the attendance of witnesses and to administer
oaths and affirmations to them. If any person shall neglect or refuse to
obey the summons of such inspector, for such purpose issued, or to testify,
except such refusal be privileged, such inspector shall certify the said
person for such neglect or refusal to the Circuit Court for the county within
which the investigation is held, and the judge thereof shall punish such
neglect or refusal as for a contempt. The costs of such investigations shall
be paid by the county within which the accident may occur, upon the war-
rant of the Chief Mine Engineer on a voucher of the district mine inspector
certifying the same.

168. In case of serious accident in or about any such mine, involving
or threatening injury to persons and property, upon receiving notice it shall
be the duty of the district mine inspector to proceed to the scene of the
accident and to direct the work of rescue, relief and restoration; and he
may request the Chief Mine Engineer to come or to detail additional
district mine inspectors to assist if necessary.

169. Persons not employees of a mine and not authorized, nor permitted
by law, shall not enter such mine until the operator has consented, nor
until and when accompanied by a competent guide authorized therefor
by such operator.
