[377.] 374. Every person to whom a certificate of registration
or OF examination is granted shall display [the same] IT
in a conspicuous part of his office [wherein the practice
of optometry is conducted]. [Every person licensed to
practice under this subtitle shall deliver, whenever he
may make] IF AN OPTOMETRIST MAKES an examination,
[prescribe or furnish] OR PRESCRIBES OR FURNISHES
spectacles, eyeglasses or lenses to any [person] PATIENT
outside of or away from his place of business, THE
OPTOMETRIST SHALL GIVE TO THE PATIENT a statement [which
shall contain] THAT CONTAINS his home address, signature
and number of his certificate or registration or OF
examination. [378.] 375. (A) Every [registered] optometrist who desires to
continue the practice of optometry in this State shall: (1)    PAY annually on [such] A date [as] SET
BY the Board [of Optometry may determine pay to the
secretary of said Board] a reasonable [registration]
LICENSE RENEWAL fee, [to be fixed by them, for which he
shall receive] SET BY THE BOARD; AND (2)    SUBMIT PROOF OF COMPLIANCE WITH THE
CONTINUING EDUCATION REQUIREMENTS. (B)    IF AN OPTOMETRIST COMPLIES WITH THE
REQUIREMENTS, THE BOARD SHALL ISSUE a renewal receipt.
[Should any ] (C)    IF AN optometrist [fail to make application
for a renewal of registration] FAILS TO COMPLY WITH THE
REQUIREMENTS, he shall be notified that his certificate
of registration OR OF EXAMINATION will be revoked within
thirty days after [such] THE notification[, and if he
fails to pay the annual fee before the expiration of the
time mentioned in said notice said Board may at once
proceed to revoke said certificate of registration]. THE
BOARD MAY REINSTATE A LICENSE REVOKED UNDER THIS SECTION
IF THE OPTOMETRIST SUBMITS PROOF THAT HE HAS COMPLIED
WITH THE CONTINUING EDUCATION REQUIREMENTS AND PAYS THE
REINSTATEMENT FEE SET BY THE BOARD. IF A LICENSE REMAINS
REVOKED FOR FIVE OR MORE YEARS, THE LICENSEE SHALL PASS
AN EXAMINATION BEFORE HE PRACTICES OPTOMETRY. [333. ] 379. (A) Any person holding a license under this
subtitle [who shall] MAY NOT attach to his name or use
the title M.D., surgeon, doctor, physician, eye
specialist, eyesight specialist, oculist,
ophthalmologist, doctor of ophthalmology, doctor of
optometry, doctor of optics, or any title containing the
